PoE 2 Boss Guide: Omniphobia, Fear Manifest Strategy & Drops
Omniphobia, Fear Manifest is a terrifying boss encountered in Journey's End during Act 4 of Path of Exile 2. Initially disguised as the NPC Freya Hartlin, this entity reveals its true form as a hulking demon of Delirium. While he serves as a major story boss in the campaign, he later returns as a much more powerful Pinnacle Boss in the endgame Simulacrum, testing players with overwhelming physical damage and sanity-draining mechanics.
Lore & Background
In the mist-shrouded lands of Act 4, you encounter a survivor named Freya Hartlin, who claims to be escaping the horrors of the fog. However, as you delve deeper into the mystery of the "Dark Mists," the facade crumbles.
Freya is merely a vessel. The entity within is Omniphobia, a manifestation of primal terror born from the Delirium. He does not just kill; he feeds on the panic of his victims. His presence in Wraeclast suggests that the boundary between reality and the nightmare realm is thinning, allowing such monstrosities to walk in the flesh.
Core Mechanics
Omniphobia is a heavy physical brawler who punishes poor positioning. His attacks are slow but hit like a truck. The fight revolves around managing Bleed and dodging his large AoE slams.
Phase 1: The Humanoid Juggernaut

1. Violent Cleave
Description: He swung the heavy weapon in his hand, sweeping across the fan-shaped area in front of him.
Strategy: Get Behind (Around the Back). This is the most basic attack; simply roll behind him. -
2. Ground Slam Fissure
Description: Omniphobia will slam the ground to create a straight line fissure that will head towards you.
Strategy: Sidestep to dodge the straight line fissure, then rush towards Omniphobia for a free hit. -
2. Leaping Ground Slam
Description: When the player is too far away, he will jump into the air and crash down to the player's position.
Strategy: Get away from the trajectory of Omniphobia's body to avoid this attack.
Special Mechanic: Visceral Vomit
At 50% Health (and frequently in the Endgame version), he gains a grotesque new ability.

Corrosive Bile
Description: Omniphobia stops moving and vomits a barrage of projectiles that leave lingering black/grey puddles on the ground.
Strategy: These puddles deal Physical and Chaos Damage over Time and slow you down. Do not step in them. The puddles last for a long time, so try to bait him to the edges of the arena to keep the center clean.
Recommended Builds
Omniphobia deals almost exclusively Physical damage, with some Chaos in the second half.
- Mercenary - Shotgun/Close Range: His animations are slow enough that you can unload a full clip into his back while he is recovering from a slam.
- Warrior - Juggernaut: High Armor and "Unstoppable" passives make the physical hits manageable. Endurance charges are very effective here.
- Key Stats: Physical Damage Reduction and Chaos Resistance are vital. A flask that cures Bleeding is mandatory.
